Privacy Policy
In accordance to the Privacy Act 1988 and the Health Records Act 2001, all information collected is treated as confidential information. We value your privacy and will use your information only for the purpose of providing you with quality care. This policy outlines our ongoing obligations to you in respect of how we manage your Personal Information.
Personal information is information or an opinion that identified an individual. For example: names, date of birth, email address, phone, Medicare number, Health Fund details and personal health information. We collect your Personal Information for 1) the primary purpose of providing our services to you, 2) secondary purposes such as marketing; or 3) in circumstances where you would reasonably expect such use or disclosure. When we collect Personal Information, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.
Sensitive information, as defined in the Privacy Act, includes information or opinion about such things as an individual’s racial or ethnic origin, criminal record, health information, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body. Sensitive information will be used by use only 1) for the primary purpose for which it was obtained, 2) for a secondary purpose that is directly related to the primary purpose, 3) with you consent; or where required or authorized by law.
AI-assisted note-taking may be included to enhance the efficiency and accuracy of clinical documentation, this practice utilises Heidi AI, an artificial intelligence tool, to transcribe and generate draft clinical notes during consultations. No audio recordings are stored—audio is processed in real-time and permanently deleted immediately after the note is generated. All data is encrypted and stored exclusively on secure, ISO27001-accredited servers located within Australia, with text transcripts automatically deleted after 30 days in compliance with Australian privacy standards. Heidi AI does not make clinical decisions or judgments, and it is used solely as a documentation aid. I, as your dietitian, remain fully and solely responsible for all clinical decisions, diagnoses, and the final content of your medical records. You will be asked for your verbal consent before each session, and you have the right to refuse or withdraw consent at any time—this will not affect your care in any way. To maintain transparency, you may request a copy of Heidi AI's full privacy policy at any time.
Third parties & disclosure of Personal Information: Where reasonable and practical to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case, we will take reasonable steps to ensure that you are made aware of the information provided to us. In some circumstances, selected Personal Information may be disclosed to other health providers if it directly related to your care i.e. medical practitioners, specialist or other allied health practitioners. Such circumstances include: 1) to third parties where you consent to the use of disclosed; and 2) where required or authorized by law.
Access, maintenance and security of Personal Information: You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access a copy of your Personal Information, please contact us in writing. We may require identification from you before releasing the requested information, in order to protect your Personal Information. Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorized access, modification or disclosure. When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most is or will be stored in client files and kept by us for a minimum of 7 years.
Payment Policy
Client and/or guardian will be solely responsible for making full payment at the time of service unless other arrangements have been made and agreed upon in writing with your dietitian at Sarah Siau Yi Yew T/A MyWellness Dietetics. Note that online consultations will require a pre-payment to secure your appointment. We accept payment via direct bank deposit or credit card (Visa, Mastercard or Amex only). If an account remains unpaid within 7 days, an administrative fee of $30 applies to each invoice.
If you have a GP Chronic Condition Management Plan or Eating Disorders Treatment and Management Plan, you will be required to make the full payment at the time of service or according to your payment plan, and we will process your Medicare rebate after each of your session.
Cancellation Policy
If you are unable to keep your appointment, please provide us with at least 24-hour notice. This allows an opportunity for other clients in need to have your appointment slot. Cancellations without adequate notice will incur a fee as below:
More than 24 hours’ notice: No fee will be charged.
Under 24 hours’ notice: A fee of 50% of the session cost will apply.
No-show: The full session fee will be charged.
*Note: medicare and private health rebates do not apply to cancellation fees. We appreciate your understanding and cooperation.
